Hello Sacred Heart Families-
I can hardly beleive it is the end of another great year at Sacred Heart of Jesus School! I hope you and your families are looking forward to a wonderful summer!
I would like to leave some information for you to look over, regarding health and wellness guidelines, for the new families joining us next year and as a reminder to our current families. It is a lot of information, but I greatly appreciate you reading through it.
Important noted from Nurse Mary:
1. Pl *Please keep children home when they are ill and until they are symptom free for at least 24 hours without the use of medication. This is our policy and is mostly non-negotiable. It is always best to err on the side of safety when potentially exposing a large group of people to a contagious illness. Please also include any pertinent information about your child’s illness, symptoms, pending tests/test results, etc. If you have any questions about our policies or procedures regarding illness, please feel free to contact me.
*Please make sure your child is drinking enough water every day. They may bring a water bottle or drink from the water fountain. Water is vital for every bodily function and so important for your child’s learning and development and can be the cause for headaches and other preventable symptoms.
3. *Please, if your child will be absent, tardy or leaving early for the day, email the attendance email (attendance@shoj.org) or myself (mary.hugerford@shoj.org), as I enter the daily attendance. I will make sure the child’s homeroom teacher is aware of any absence.
4. * At the beginning of every school year, I need;
a. a student health assessment form for every student (please make sure these forms are filled out by a parent and as accurately and detailed as possible) and an emergency information and medical treatment consent form, one per family.
b. for new students (all incoming kindergartners and all transfer students), in addition to the two above forms, I need a physician signed physical and a copy of the students most recent immunization record (make sure the record is clearly marked with the student’s name).
c. if a student has a food allergy, asthma, seizure disorder or diabetes, I will need a form for each of those diagnosis.
d. all students entering 7th grade will need to turn in an updated immunization record showing that they have received the required Tdap and meningococcal vaccines.
*Each of these forms is on the school website (school.shoj.org), under documents>enrollment>medical. I also have copies in my office. I would greatly appreciate all forms be filled out, signed and turned in on/before enrollment day.
5. *If your student will need to be given a prescription medication during the school day, I will need the medication brought to me in a prescription bottle/box with the student’s name, medication, dose and time to be given. This includes inhalers and epi pens, for use as needed. Please also make sure prescription medications are not expired. I will not administer unlabeled prescription medications.
6. *If there is a need to give a non-prescription medication that I do not have in the nurse’s office, please give it to your child at home before school. If there is a situation where there will be a need to give a non-prescription medication during school hours, that I do not have on hand, before sending it in, please reach out to me to discuss. Refer to the emergency information and medical treatment consent form for the list of non-prescription medications that are available in the nurse’s office.
7. ** Please make sure your child is eating breakfast, with an adequate amount of protein, each morning, especially on mass days and testing days.
8. *If your child cannot participate in PE for any illness or injury, we require a note. If the student has had an injury and comes in a brace or ace wrap, we need a doctor’s note regarding activity restrictions and when they can return to full play/PE.
