Hello Sacred Heart families!

I know it is almost just summer (yay!), but I wanted to send out some information now, as I know how busy life can get and before we know it another school year will be around the corner!

-At the beginning of every school year, I need a student health assessment form for each student (please make sure these forms are filled out by a parent and as accurately and detailed as possible) and an emergency information and medical treatment consent form, one per family. For new students (all incoming kindergartners and all transfer students), in addition to the two above forms, I need a physician signed physical and a copy of the students most recent immunization record (make sure the record is clearly marked with the student’s name). If a student has a food allergy, asthma, seizure disorder or diabetes, I will need a form for each of those diagnosis. All students entering 7th grade will need to turn in an updated immunization record showing that they have received the required Tdap and meningococcal vaccines. Each of these forms is on the school website (school.shoj.org), under documents. I also have copies in my office. I would greatly appreciate forms be filled out, signed and turned in on/before enrollment day.

-If your student will need to be given a prescription medication during the school day, I will need the medication brought to me in a prescription bottle/box with the student’s name, medication, dose and time to be given. Please ensure the medication is not expired. This includes as needed medications, such as inhalers and epi pens. I will not administer unlabeled prescription medications. If there is a need to give a non-prescription medication that I do not have on hand in the nurse’s office, please give it to your child at home before school. If there is a situation where there will be a need to give a non-prescription medication during school hours, that I do not have on hand, before sending it in, please reach out to me to discuss. Refer to the emergency information and medical treatment consent form for the list of non-prescription medications that are available at school.
